Step 1:
Complete the Intake Forms
To begin the supervised visitation process, you’ll need to complete a few short forms to help us understand your family's needs and comply with court requirements.
✔ Fill out the Parent/Guardian Intake Form (15–20 minutes)
✔ Upload required documents:

Court order or visitation agreement

Valid photo ID
✔ Submit a Child Information Form for each child participating in visits

Submit the Intake Fee ($150)
A one-time, non-refundable $150 intake fee is required before we can schedule your intake appointment.
We offer multiple payment options for your convenience:
✔ Online Payments – Secure payments through our website
✔ Cashier’s Check or Money Order – Payable to Roots on the Road.
✔ Cash Payments – Accepted in person at our office
This fee must be paid in full before the intake appointment can be scheduled.

Step 3:
Schedule Your Intake Appointment
Once we receive your completed forms and payment, our team will contact you within 48 business hours to schedule your intake appointment.
At your appointment, we will:

Step 4:
Begin Visits & Ongoing Reporting
Once your intake is complete and approved, supervised visits can begin based on your court order and our availability.

✔ A trained supervisor is present at all times
✔ Reports are documented for each visit and may be shared with the court or GAL (if required)

At Roots on the Road, we believe in transparent pricing so you can plan ahead for your supervised visitation needs. Below, you will find a detailed breakdown of the costs associated with our services, along with the available payment options.
Supervised Visitation Fees
Intake Fee: $150 (One-time fee, required before scheduling visits)
Visitation Fees:
$75 per hour for 1-2 children
$100 per hour for 3 or more children
Drop-Off/Pick-Up Supervision Fee: $50 per event ($25 per parent)
Therapeutic Visitation Fee: $100 per hour
Court Appearance Fee:
$50 per hour (standard facilitator)
$75 per hour (therapeutic facilitator)
What Does the Intake Fee Cover?
The one-time intake fee of $150 includes:

A required intake meeting to review court orders, documentation, and expectations.

Administrative processing of intake forms and required paperwork.

Establishing a visitation plan tailored to your court orders and family needs.
This fee must be paid in full before the intake appointment can be scheduled.
When Are Payments Due?

Outstanding balances over 15 days will result in a temporary suspension of services until the account is paid in full.
All payments must be made prior to or at the time of service.

Who needs supervised visitation?

How often can visits occur?
Visit frequency depends on the court order and availability. Parents can request specific days and times, but final scheduling must align with facilitator availability and legal guidelines.
What happens if I need to cancel a visit?

For illness: Notify us at least 24 hours in advance to avoid additional charges.

For other reasons: Notify us at least one week in advance.
Failure to provide notice may result in a $50 penalty fee.
What happens if a parent does not show up for a scheduled visit?
A missed visit without prior notice will be documented and may be reported to the court. Parents who frequently miss visits may face further legal consequences.
Visitation Rules & Conduct
Can I bring gifts for my child during the visit?
Gifts are only allowed for birthdays or court-approved holidays. All gifts must be pre-approved by the facilitator before being given to the child.
Can I bring food or snacks for my child?
Yes, but with some restrictions:

Meals must be nutritious and age-appropriate.

Takeout meals are limited to once per month.
-
If the visit lasts more than two hours, parents are expected to provide a snack or meal.
Are electronic devices allowed during the visit?
No cell phones, tablets, or recording devices are allowed during supervised visits, unless required for legal or medical reasons.
What if the visiting parent or child is sick?
For the health and safety of all parties, no visits will take place if the child or visiting parent is experiencing:

Cough, cold, or flu-like symptoms

Stomach illness (nausea, vomiting, diarrhea)
A doctor’s note may be required for repeated missed visits due to illness.
Legal & Confidentiality
Will reports be shared with the court?
Our staff maintains detailed records of all supervised visits. These reports will not be shared with either parent unless:

A Guardian Ad Litem (GAL) or attorney of record formally requests documentation.

A safety or legal concern requires reporting.
Can other family members attend the visit?
Only approved individuals listed in court orders or pre-approved by the custodial parent may attend supervised visits. Requests for additional attendees must be submitted at least 72 hours in advance.
